Trans Abstract

Treatment of diabetic dyslipidemia is important for prevention of cardiovascular disease in patients with diabetes. Although glucose control is the main target in patients with diabetes to prevent diabetic complications, multifactorial interventions are absolutely important in reducing risk for cardiovascular disease. Recent joint guidelines from European Society of Cardiology and European Society for the Study of Diabetes recommended lower low-density lipoprotein cholesterol (LDL-C) targets for patients with diabetes and included diabetes duration in the stratification of risk groups for cardiovascular disease prevention, emphasizing higher cardiovascular risk in patients with diabetes. In addition, American Heart Association/American College of Cardiology guidelines were revised in that the LDL-C cutoff actually appeared in the guidelines for high risk groups and for the change of treatment options. The Korean Diabetes Association released new treatment guidelines for patients with diabetes and adopted recent changes from other guidelines with respect to dyslipidemia control. In this review, I would like to review recent updates in guidelines regarding dyslipidemia treatment for patients with diabetes.

서론

당뇨병은 다양한 혈관합병증으로 인해 사망률과 심혈관질환 위험도가 높은 만성질환이다. 당뇨병 환자의 경우 비당뇨인에 비해서 심혈관질환에 의한 사망률이 2∼4배 높기 때문에, 당뇨병 환자에서 심혈관질환 위험인자의 조절은 매우 중요하다[1]. 당뇨병 환자는 혈당조절뿐 아니라 혈압, 이상지질혈증, 체중 관리, 및 금연 등 다양한 인자들의 조절이 필수적이다. Steno-2 연구에서는 당뇨병 환자에서 다양한 심혈관 위험인자의 조절이 심혈관질환 발병을 53% 줄일 수 있는 것으로 보고되어, 당뇨병 환자에서의 이러한 위험인자의 조절이 매우 중요한 시점이다[2].

이상지질혈증은 당뇨병 여부를 막론하고, 동맥경화증과 심혈관질환의 가장 큰 위험인자로 알려져 있다. 본 글에서는 당뇨병 환자에서의 이상지질혈증의 위험도 및 현황을 살펴보고 이를 치료하기 위한 연구들과 최근 가이드라인에서 제시하는 이상지질혈증 치료에 대한 부분을 살펴보고자 한다.

당뇨병 환자에서 이상지질혈증

2018년 대한당뇨병학회 Diabetes Fact Sheet에 의하면 2013∼2016년 국민건강영양조사 결과를 토대로 분석 한 결과, 총콜레스테롤 240 mg/dL 이상이거나 약제를 복용하는 것으로 정의한 고콜레스테롤혈증의 비율은 30세 이상 한국인 당뇨병 환자에서 34.9%로 나타났으며, 조절률은 44.2%로 보고되었다[3]. 한국지질동맥경화학회 Dyslipidemia Fact Sheet에서는 이상지질혈증을 좀 더 넓은 의미로 정의하였음에도 당뇨병 환자에서 비당뇨병 환자보다 약 2.3배 정도 이상지질혈증의 유병률이 더 높음을 보여주었다[4]. 또한 저밀도지단백 콜레스테롤(low-density lipoprotein cholesterol, LDL-C) 100 mg/dL 미만을 목표치로 했을 때의 조절률은 44.2%로 분석되었다. 이상에서, 한국인 성인 당뇨병 환자 10명 중 3명 이상이 고콜레스테롤혈증을 가지고 있으며, 10명 중 4명만이 목표치에 도달하도록 조절함을 알 수 있어, 좀 더 적극적인 치료가 필요함을 시사하였다.

제2형 당뇨병은 복부비만으로 인한 인슐린저항성이 주된 병리기전인데, 인슐린의 지방분해 억제 효과가 줄어들면서 지방분해가 증가하고 이로 인해 지방세포에서 유리지방산이 혈중과 간으로 더 많이 유입된다. 이로 인해 증가한 중성지방이 콜레스테롤 에스터와의 교환으로 hepatic lipase에 의한 고밀도지단백 콜레스테롤(high-density lipoprotein cholesterol, HDL-C)의 축소, 분해를 조장, 신장으로 배설을 유도하고, LDL-C은 작고 밀도가 높아지게 되는데(small, dense; sdLDL-C), 이러한 sdLDL-C는 subendothelial space로 더 잘 침투하여 동맥경화증을 더 잘 일으키는 것으로 알려져 있다[5]. 즉, 제2형 당뇨병 환자에서의 이상지질혈증은 높은 중성지방, 낮은 HDL-C, 그리고 높은 sdLDL-C 농도의 특징을 가지게 되고 이로 인해 제2형 당뇨병 환자는 동맥경화증에 취약하게 된다.

Statin을 이용한 기존의 많은 심혈관질환 예방연구들은 한마디로 “The Lower, the better”의 결과들을 보여준다[6]. 고지혈증 치료지침의 족보라 할 수 있는 Adult Treatment Panel (ATP) I에서 처음으로 LDL-C를 심혈관질환 예방을 위한 목표로 잡을 것을 제안하였고, 이후 대부분의 가이드라인이 LDL-C를 낮추는 것을 목표로 하는 가이드라인을 제시하였으며, 많은 연구들이 LDL-C를 낮추면 낮출수록 심혈관질환 위험도가 감소하는 결과를 보여주었고, 이는 당뇨병 환자에서도 예외는 아니었다[7].

제2형 당뇨병의 대표적인 합병증 예방 중재 연구인 United Kingdom Prospective Diabetes Study (UKPDS)에서 관상동맥질환 발병의 위험인자를 한 모델에 넣어서 분석한 결과, 관상동맥질환의 위험에 가장 영향을 미친 인자는 당화혈색소가 아닌 LDL-C 농도인 것으로 분석되었고, 두 번째가 HDL-C였다[8]. 이 결과는 당뇨병 환자에서도 LDL-C를 목표치에 도달하도록 낮추는 것이 심혈관질환을 예방하는 데 가장 중요하다는 것을 시사하며, LDL-C를 1 mmol/L 낮추었을 때 관상동맥질환을 36% 낮출 수 있다고 하였다.

최근 Swedish National Diabetes registry를 이용해서 271,174명의 제2형 당뇨병 환자와 1,355,870명의 정상인을 match해서 분석한 결과를 보면, 사망률이나 뇌졸중 등과는 달리 관상동맥질환은 역시 LDL-C와 매우 선형의 상관성을 보여주고 있어 LDL-C를 목표치에 도달하도록 낮추는 것이 매우 중요함을 알려주고 있다[9].

제2형 당뇨병 환자에서의 이상지질혈증 관련 임상연구

당뇨병 환자에서 고지혈증 치료와 심혈관질환과의 연관성을 본 1차 연구가 많지는 않은데, 대표적인 연구는 Collaborative Atorvastatin Diabetes Study (CARDS) 연구이다[10]. 관상동맥질환 병력이 없고 LDL-C은 160 mg/ dL 이하인 2,838명의 제2형 당뇨병 환자를 atorvastatin 10 mg군과 위약군으로 나누어 1차 효과 변수로 심혈관질환 발병시점을 확인해보고자 하였다. 이 연구는 거의 2년 가량 빨리 종료되었는데, 이는 매우 뚜렷한 atorvastatin 의 심혈관 보호효과가 연구 초기부터 나타났기 때문이었다. Atorvastatin 10 mg을 투여받은 군에서 위약군에 비해 심근경색이 42%, 뇌졸중이 48% 낮게 관찰되었다. CARDS 연구는 이후 대부분의 가이드라인에서 당뇨병 환자에서 statin과 LDL-C 저하가 심혈관질환 예방에 미치는 효과의 가장 중요한 연구로 언급되고 있다.

이후 statin 임상연구들을 모아서 분석한 Cholesterol Treatment Trialists’ Collaborators 연구에서는, 총 18,686명의 당뇨병 환자와 71,370명의 비당뇨병인에서 LDL-C 감소에 따른 심혈관질환 위험도 감소 효과를 분석하였다[11]. 이 연구에서는 LDL-C의 1 mmol/L 감소시 사망율은 9%, 심혈관질환은 21%의 감소 효과가 있었다. 이는 당뇨병이 없는 대상자에서와 비슷한 감소 효과였으며 이전의 심혈관질환 유무와는 별개로 나타나서, 당뇨병 환자에서도 statin 투여를 통한 LDL-C 감소가 심혈관질환 예방에 매우 중요함을 증명하였다.

최근 가이드라인들이 statin 중심의 치료만을 권고하고 있던 중, 소장에서 콜레스테롤의 흡수를 억제하는 ezetimibe 제제가 시장에 나왔다. Ezetimibe는 소장벽의 외피세포와 간세포에서 콜레스테롤을 흡수를 담당하는 Niemann-Pick C1-like 1 (NPC1L1) 단백질을 차단함으로써 대변으로 콜레스테롤이 나가도록 하는 작용을 하여 혈중 콜레스테롤을 낮춘다[12]. 18,144명의 급성관상동맥증후군 환자에서 ezetimibe의 효과를 보았던 Improved Reduction of Outcomes: Vytorin Efficacy International Trial (IMPROVE-IT) 연구는 simvastatin 40 mg/ezetimibe 10 mg과 simvastatin 40 mg을 투여한 군으로 나누어 심혈관질환 발생을 비교하였다[13]. 6년간 추적관찰한 결과, 연구 종료시 평균 LDL-C 농도는 simvastatin/ezetimibe군에서 53.7 mg/dL, simvastatin군에서 69.5 mg/dL였는데, ezetimibe를 추가적으로 투여한 군에서 1차 효과 변수인 심혈관질환을 통계학적으로 의미있게 6% 가량 더 낮출 수 있어서 statin을 투여하는 환자에게 ezemitibe의 추가적인 투여가 효과가 있음을 증명하였다. 또한 이 연구에 포함된 4,933명의 당뇨병 환자만을 대상으로 따로 분석한 결과를 보면, 당뇨병 환자에서 비당뇨병 대상자보다 이 약제의 효과가 더 높은 것으로 분석되었다[14]. 이 연구의 결과가 ezetimibe를 statin 다음 단계로 추가할 약제로써 가이드라인에 권고될 수 있도록 하는 근거가 되었다.

최근 가이드라인에서 치료법의 가장 큰 변화는 statin, ezetimibe 다음 단계로 Proprotein convertase subtilisin/ kexin type 9 (PCSK9) 억제제의 권고이다. PCSK9은 proprotein convertase family의 하나로, LDL 수용체에 결합하여 degradation을 시키는 작용을 한다[15]. PCSK9 억제제는 PCSK9의 이러한 작용을 억제하여 LDL 수용체를 간세포 및 다른 세포 표면에 더 많이 발현시킴으로써 혈중 LDL-C 농도를 낮춘다. 최근 Further Cardiovascular Outcomes Research With PCSK9 Inhibition in Subjects With Elevated Risk (FOURIER) 연구 등 PCSK9 억제제 연구들에서, statin을 이미 쓰고 있는 환자들에게 추가했을 때 추가적으로 LDL-C를 매우 낮게 낮추며, 심혈관질환 위험도도 추가적으로 낮춤으로써, 이 약제의 심혈관질환 예방 효과를 입증하였다[16,17]. 또한 최근 발표된 당뇨병 환자에서의 하위분석에서도, 당뇨병이 기저에 있었던 환자들에서 없었던 환자들보다 더 나은 결과를 보여, 당뇨병 환자에서 이 약제의 추가를 권고하게 되었다[18].

이처럼, LDL-C를 낮추는 약제들의 선전과는 반대로, 중성지방을 겨냥한 약제들은 고전을 면치 못했다. 대표적인 중성지방 저해제인 fibrate는 당뇨병 환자를 대상으로 했던 Fenofibrate Intervention and Event Lowering in Diabetes (FIELD) 연구와 Action to Control Cardiovascular Risk in Diabetes (ACCORD) 연구에서 유의한 심혈관질환 예방 효과를 보이지 못했으나, ACCORD 연구에서는 중성지방이 높고 HDL-C이 낮은 하위그룹에서 경계 정도의 효과를 보여서, 적극적으로 가이드라인에 권고되지 못했다[19,20]. 또 다른 중성지방 저해제인 omega-3 역시 유의한 예방효과를 보이지 못하다가, 최근 발표된 REduction by DUtasteride of prostate Cancer Events (REDUCE-IT) 연구에서는 eicosapent ethyl 성분만으로 투여했을 때, 25%의 유의한 심혈관질환 예방효과를 보였고, 당뇨병 환자만 분석한 하위분석에서도 유의한 예방효과를 보였다[21]. 그러나 이는 한 연구에서만 보인 결과이며, eicosapent ethyl 성분만을 투여한 결과라 아직 논란이 있겠다.

해외 및 타학회 진료지침

이상지질혈증 치료기준 개발의 역사는 National Cholesterol Education Program Expert Panel on Detection, Evaluation, and Treatment of High Blood Cholesterol in Adults-Adult Treatment Panel (NCEP-ATP)의 역사와 함께 한다. 1988년에 발표된 ATP I에서는 심혈관질환 예방을 위해서는 LDL-C 농도를 낮추는 데 중점을 두어야 한다고 강조했고, 1993년에 발표된 ATP II에서는 위험인자 및 위험률에 따른 위험군으로 나누어서 각각 다르게 치료해야 함을 권고하였다. 2001년에 발표된 ATP III에서는 고위험군에서는 LDL-C 농도가 100 mg/dL 미만이어야 함을 강조했고, 관상동맥질환 equivalent군에 당뇨병이 포함되어 고위험군으로 분류되었다. 2004년에 업데이트되어 발표한 ATP III에서는 대사증후군이 초고위험군으로 분류되고, 심혈관질환이 있는 당뇨병은 초고위험군으로 분류되어 LDL-C 70 mg/dL 미만까지 고려할 수 있다고 명시하였다[7].

ATP IV를 기다리던 근 10년의 시간 후, 2013년에 American College of Cardiologist/American Heart Association (ACC/AHA) 가이드라인이 발표되었다[22]. 이 가이드라인에서는 LDL-C의 목표치를 없애고, 모든 대상자 중 1) 심혈관질환을 가진 환자, 2) LDL-C 190 mg/ dL 이상, 3) 40∼75세의 당뇨병 환자, 4) 심혈관질환 위험도가 7.5% 이상인 40∼75세의 환자를 4개의 statin benefit 그룹에 포함시켰다. 이 criteria에 해당되는 경우 무조건 moderate intensity 이상의 statin을 쓰도록 권고하였고, 각 statin을 high, moderate, low intensity의 세 군으로 분류하여 적절한 statin을 사용하도록 하였다. 그러나 이 가이드라인은 종족 간의 차이를 고려하지 않았고, 언급된 연구들이 주로 백인에서 이루어진 연구들이며, LDL-C의 목표치를 없애고 비스타틴 치료에 대한 권고가 거의 전무하다는 파격적인 가이드라인으로서 많은 호응을 얻지는 못했다.

2016년도 발표된 European Society of Cardiology (ESC) 가이드라인은 2013년 ACC/AHA 가이드라인을 답습하지 않고 기존의 위험군 분류를 그대로 사용하였다[23]. 표적장기 손상을 가진 당뇨병 환자는 초고위험군으로, 표적장기 손상이 없는 당뇨병 환자는 고위험군으로 분류하였고, 초고위험군 환자는 LDL-C 목표치를 70 mg/dL, 고위험군은 100 mg/dL 미만으로 조절할 것을 권고하였다. 지난 해 발표된 2019년 ESC 가이드라인에서는 유병기간이 20년 이상인 제1형 당뇨병을 초고위험군으로 분류하였고, 10년 이상인 당뇨병 환자는 고위험군으로 분류하여 당뇨병 유병기간에 따라 비교적 논리적으로 위험군을 설정한 점이 특징적이다(Table 1). 또한 이번 가이드라인에서는 LDL-C 목표치를 더 낮추어서, 초고위험군은 55 mg/dL까지, 고위험군은 70 mg/dL 미만까지 낮추도록 권고하여 당뇨병 환자에서의 목표치가 더 낮아지게 되었는데 이는 당뇨병 환자의 위험도가 더 높아졌음을 시사한다(Table 2). LDL-C이 주된 목표이며 주된 치료방법은 statin이지만, 목표에 도달하지 않거나 부작용이 있을 시에는 ezetimibe, 그리고 PCSK9 억제제까지 투여하도록 권고한다. 재미있는 것은, 앞서 언급한 REDUCE-IT 연구 결과를 토대로 statin을 투여해도 중성지방이 높은 환자에서는 eicosapent ethyl 2 g 매일 2회를 권고수준 IIa로 권고하고 있다는 것이다[21].

2019년 다시 발표된 ACC/AHA 가이드라인에서는 1차 예방을 위한 권고안에 다소 많은 변화를 주었는데, 당뇨병 환자에서 위험 가중요인(risk enhancer)을 제시하였는데, 이는 유병기간이 긴 당뇨병, 알부민뇨증, estimated glomerular filtration rate 60 mL/min/1.73 m2 미만, 망막병증, 신경병증, 발목상완지수 < 0.9 등의 위험요인이 있을 때 high intensity statin을 쓰도록 권고하였다[24]. 또한 치료에 있어서도 적절한 statin 투여에도 LDL-C 70 mg/dL 에 도달하지 않으면 다음 치료를 추가할 수 있다는, 즉 실제적인 LDL-C 목표치가 다시 출현하였다. 미국당뇨병학회 가이드라인은 위의 2013, 2019 ACC/AHA 가이드라인에서 당뇨병 환자 관련 내용을 그대로 정리해서 권고하고 있다(Table 1). 역시 주된 치료는 statin 그리고 ezetimibe와 PCSK9 억제제의 순으로 권고하고 있다(Table 2).

한국지질동맥경화학회 치료지침은 2018년에 4판을 발표하였는데, 1996년에 1판을 발간한, ATP 가이드라인과 거의 비슷한 매우 역사가 긴 치료지침이다[25]. 이 지침에서는 당뇨병은 고위험군에 속해 있으며, 표적장기 손상이 있거나 중요한 심혈관질환 위험인자를 동반한 경우 초고위험군으로 격상할 수 있다고 명시하였다(Table 1). 또한 초고위험군은 LDL-C 100 mg/dL, 고위험군은 70 mg/dL까지 낮추도록 권고하였다(Table 2).

대한당뇨병학회 진료지침 제6판

앞에서 많은 가이드라인을 설명했지만, 아래의 대한당뇨병학회 가이드라인을 참조하는 것이 가장 간단할 것으로 생각된다[26].

  • 1. 혈청 지질검사(총콜레스테롤, HDL-C, 중성지방, LDL-C)는 당뇨병 진단 시 그리고 매년 1회 이상 시행한다. 약제 시작 전과 4∼12주 후 검사를 하여 투약에 대한 반응 및 순응도를 평가한다. [E, I]

  • 2. 심혈관질환이 없는 당뇨병 환자는 LDL-C를 100 mg/ dL 미만으로 조절한다. [A, I]

  • 3. 심혈관질환이 있는 당뇨병 환자는 LDL-C를 70 mg/dL 미만으로 조절한다. [A, I]

  • 4. 알부민뇨, 만성신질환(사구체여과율 60 mL/min/1.73 m2 미만) 등의 표적장기 손상이나 고혈압, 흡연, 관상동맥질환 조기발병 가족력 등의 위험인자를 가지고 있는 당뇨병 환자는 LDL-C를 70 mg/dL 미만으로 조절한다. [B, IIa]

  • 5. 중성지방의 조절 목표는 150 mg/dL 미만, HDL-C는 남자에게서 40 mg/dL, 여자에게서 50 mg/dL 초과이며, 이를 위해 생활습관을 적극적으로 교정하고 혈당조절을 엄격히 한다. [C, I]

  • 6. 이상지질혈증이 있는 경우 적극적인 생활습관교정을 하도록 교육한다. [A, I]

  • 7. 당뇨병 환자의 이상지질혈증에서는 statin을 일차치료약제로 사용한다. [A, I]

  • 8. 심혈관질환이 있는 당뇨병 환자에게서 최대내약용량(maximal tolerable dose)의 statin으로 LDL-C 목표치에 도달하지 못한 경우 ezetimibe나 PCSK9 억제제의 추가를 고려한다. 단, 비용을 고려하여 ezetimibe를 선호한다. [B, IIa]

  • 9. 심혈관질환이 없는 당뇨병 환자에게서 최대내약용량의 statin으로 LDL-C 목표치에 도달하지 못한 경우 ezetimibe의 추가를 고려할 수 있다. [C, IIb]

또한 대한당뇨병학회 제6판 진료지침에서는 당뇨병 환자에서의 이상지질혈증 약물치료전략을 그림으로 자세히 설명하고 있다(Appendix 1) [26]. 먼저, 심혈관질환 존재 여부, 표적장기 손상 여부 혹은 주요 심혈관질환 위험인자가 있는지를 파악하여 LDL-C 목표치를 설정한다(Table 1). 기저 LDL-C이 목표치에 도달하지 않은 경우 statin 투여를 우선 시작하고, 투여시작 4∼12주 후 LDL-C검사를 시행하여 목표치에 도달했는지 그리고 부작용은 없는지 파악한다(Table 2). 목표치에 도달했으면 기존 statin 치료를 유지하고, 도달하지 못했거나 부작용이 있으면 다른 치료방법의 병합 혹은 치환을 고려한다.

결언

본 종설에서 다룬 내용을 Tables 1, 2로 정리해 보았다. 복잡해 보이지만, 당뇨병 환자에서의 LDL-C 목표치는 70 mg/dL까지, 그리고 위험도가 높은 환자에서는 55 mg/dL 까지 권고하고 있지만, 아직 여기에 대한 증거는 많지 않다. 치료는 statin이 첫 번째, 그 다음으로는 ezetimibe와 드문 경우에는 PCSK9 억제제까지 고려할 수는 있다. PCSK9 억제제는 현재 보험이 되지 않아 투여는 매우 제한적인 환자에서만 가능할 것이다. 중요한 것은, 당뇨병 환자에서의 심혈관질환 위험도가 높고, 따라서 당뇨병 환자에서의 이상지질혈증 치료가 적절히 이루어지는 것이 심혈관질환의 예방 및 치료에 매우 중요하다는 점이다. 이상지질혈증 치료율이 100%가 되지 않는 부분이 열심히 치료해도 심혈관질환으로 사망하는 위험도가 아직 높다는 사실에 기여를 하고 있을 가능성을 인지하고, 적절한 위험도 평가 후 적극적으로 LDL-C을 목표치에 도달하도록 치료하는 것이 중요하겠다.
