Trans Abstract

Abstract

Aging and chronic disease are often accompanied by changes in body composition that lead to decreased muscle mass and increased fat mass, even in weight-stable individuals. Both obesity and sarcopenia are important causes of frailty, disability, morbidity, and mortality. Diminished skeletal muscle and expanded visceral fat can act synergistically, which might maximize their effects on physical impairments and metabolic disorders. Dual-energy X-ray absorptiometry (DEXA) was regarded as one of the most versatile imaging techniques for evaluation of sarcopenia and obesity as well as osteoporosis. Whole-body DEXA allows total and regional assessment of the three compartments for fat mass, non-bone lean mass, and bone mineral content. Moreover, DEXA is accurate, reproducible, fast, relatively inexpensive, and involves very low radiation dose to the patient. Developments in DEXA equipment and software allow an accurate and differential estimate of visceral adipose tissue. This review summarizes the clinical practical application of whole-body DEXA values, with use of muscle and fat mass indices in the diagnostic workup of low muscle mass, sarcopenia, and sarcopenic obesity according to the most recent studies and guidelines.

서론

비만과 인구의 고령화 문제는 우리나라만의 문제가 아니라 21세기 전 세계의 화두이다. 중년 이후 연령 증가와 비만은 각각 신체구성성분의 변화와 이로 인한 만성질환의 이환과 악화에도 중요한 역할을 하고 있다[13]. 지난 수십 년 동안 비만의 유병률은 전 세계적으로 급격하게 증가하고 있고 우리나라 또한 2009년에서 2018년 사이에 체질량지수(body mass index, BMI) 25.0 kg/m2 이상의 비만 유병률이 2009년에 32.6%에서 2018년에 38.5%로 18% 증가하였다[4]. 비만 유병률은 전 연령대에서 증가추세를 보여주었고 특히 80세 이상의 노인에서 두드러지게 증가하였다[5]. 비만을 측정하기 위해 사용되는 BMI는 체지방률과 상관도가 높아 비만의 지표로 널리 사용하고 있다. 그러나, 제지방량이 감소하고, 체지방량이 증가하는 노인에 있어 BMI만으로 비만을 평가한다면 체지방량을 과소평가할 수 있을 것이다.

비만은 건강을 위해할 정도의 과도한 지방축적으로 정의하고 있으나[6], 최근 여러 연구에서는 일반 성인에서 BMI 의 비만 진단기준이 노인에서는 적합하지 않다고 하여 새롭게 근감소성 비만(sarcopenic obesity)이라는 개념을 제시하고 있다[79]. 근감소성 비만은 과도한 지방량의 축적상태에서 제지방량이 감소한 상태를 의미하며[10], 연령이 증가함에 따라 그 유병률과 위험이 증가한다고 보고하고 있다[8,11]. 노인에서 비만은 근감소증(sarcopenia)을 악화시키는데, 근육 내에 지방침윤을 증가시키고, 신체활동능력을 저하시키며, 사망 위험 또한 증가시킨다[1215]. 65세 이상의 노인인구비가 급격하게 증가하고 있고 노인인구에서의 비만 유병률 또한 지속적으로 늘어나고 있어 비만의 예방과 치료는 개인만의 문제가 아닌 국가적인 차원에서 관리가 필요하다. 특히, 노인 비만의 효과적인 관리를 위해서는 먼저 정확하게 진단하는 것이 매우 중요할 것이다.

비만 정도를 평가하기 위한 방법의 신뢰도와 타당도를 평가할 때 신체의 구조적 틀을 구획별로 나누어 평가하는 신체구획모델개념을 적용하게 된다[16]. 두 구획모델(two-com-partment model)은 전신을 지방량(fat mass)과 제지방량(fat-free mass)으로 나누고, 제지방량은 다시 화학적으로 수분, 단백질 및 무기질로 구성되므로 전신 2-구획모델은 화학적 4-구획모델로 확장하게 된다[17]. 비만을 진단하기 위해 BMI, 허리둘레, 피부주름두께 등의 신체계측법을 이용할 수 있고 생체전기저항측정법(bioelectrical impedance analysis, BIA), 이중에너지방사선흡수측정기(dual-energy X-ray absorptiometry, DEXA), 자기공명영상(magnetic resonance imaging, MRI), 컴퓨터단층촬영(computed tomography, CT), 공기전위 체적변동측정기(air displacement plethysmography) 등의 기계계측법을 활용할 수 있다(Table 1). 신체계측법들과 BIA는 비교적 간편하여 다수의 인원을 대상으로 비만이나 과체중 여부를 진단하는 데 사용할 수 있는 장점이 있으나, 신체계측법은 근육량과 지방량을 산출하는 데 한계가 있다[18,19]. 정확한 신체구성요소를 측정하고 체지방이나 근육량의 인체 분절별 분포 양상을 측정할 수 있는 방법으로 CT 또는 MRI를 사용할 수 있으나, CT 는 방사성 노출의 위험이 높고 비용이 많이 든다는 단점이 있다[20,21]. MRI는 CT와 같이 내장지방과 피하지방을 명확히 분리하여 측정할 수 있으면서 CT와 달리 방사선 노출이 없다는 장점이 있으나, 높은 비용과 접근성의 문제가 있다. 이에 반해 DEXA는 체지방에 대한 표준측정법으로 임상연구에서 활발하게 이용하여 왔고, 근육량측정에 있어 CT나 MRI로 측정한 근육량과 높은 상관계수를 보이며 방사선 노출량이 적다는 장점으로 인해 사지근육량측정의 표준검사로도 인정되고 있다. 최근 국내에서는 근감소증에서의 DEXA를 이용한 체성분분석이 2019년 신의료기술평가위원회에서 근감소증 의심환자 및 근감소증 환자를 대상으로 진단 및 치료효과를 모니터링하는 데 있어 안정성 및 유효성이 있는 기술로 인정받아 임상에서도 널리 활용할 수 있는 길이 열렸다.

Methods for assessing the degree of obesity

DEXA의기본원리

X선 영상은 특정한 에너지에 해당하는 광자(photon)가 시작단(X-ray source)으로부터 방출되어 장애물에 의해 흡수 혹은 산란된 후 최종 검출단에 도달하는 광자의 양에 대응되는 밝기 값으로 표현되는 영상이다(Fig. 1) [22]. DEXA는 체성분분석의 기준이 되는 기법으로 지방량, 제지방량 및 골무기질 함량의 측정을 기본으로 하는 세 구획모델로 평가한다(Fig. 2). 두 개의 에너지 수준에 차이가 있는 저용량의 방사선이 인체를 투과할 때 투과물질의 방사선 투과율을 측정하는 것이 기본 원리이다. X선 빔 에너지는 조직을 투과할 때 감쇠되며, 에너지 강도와 조직의 두께와 밀도에 영향을 받게 된다. 즉, 광자 에너지가 증가할수록 X선 빔 감쇠 정도는 낮아지게 되고, 저밀도 물질(예: 연조직)에서는 광자가 훨씬 잘 투과하게 되어 뼈와 같은 고밀도 물질보다 감쇠 정도가 낮게 나타난다. 두 개의 X선 에너지 최고점에서의 감쇠 차이는 각 조직에 따라 다르다. 따라서 DEXA는 두 개의 서로 다른 에너지 최고점에서의 감쇠계수 비율(R-value)을 측정한다. R-value 는 모든 검사대상자에서 뼈와 지방에 대해서는 일정하지만 연조직의 R-value는 항상 다르며, 대상자의 연조직 구성에 따라 다르게 나타난다(낮은 R-value는 높은 지방지율을 의미한다).

DEXA가 전신 또는 부위별 지방량, 제지방량 및 골무기질 함량을 제공하지만, 세 가지 구성요소를 모두 직접 측정하지는 않는다. 전신 스캔의 경우, pixel의 약 40~45%가 뼈에 포함되어 있는데, 뼈에 인접한 연조직도 포함할 수 있다. DEXA 로 측정된 영상에서 pixel은 연조직 pixel과 뼈 pixel로 분류하여 골밀도 값과 체지방량을 계산하며, 뼈 pixel은 연조직량과 골량의 조합으로 이루어져 있다고 가정한다. 연조직 pixel의 구성성분은 지방조직과 뼈를 제외한 제지방조직으로 이루어져 있다고 전제로 하고, 일차적으로 연조직 pixel의 체지방량을 구한다. 뼈 pixel의 연조직량 또한 연조직 pixel 과 같은 체지방량을 가진다고 가정하고, 이차적으로 뼈 pixel 에서 연조직 pixel의 특정 감쇠 성분량을 이용해 골량을 구하고, 연조직량 또한 구할 수 있다.

DEXA활용법

체성분의 정확한 측정은 생리적 또는 다양한 임상 상황에서의 에너지대사를 이해하는 데 도움을 주기에 유용하다. 만성질환과 악화는 뼈와 근육을 포함한 체성분에 영향을 미치며, 이론적으로 이러한 변화는 모든 질병에서 나타날 수 있다. 따라서 전신 DEXA 검사를 통한 포괄적인 체성분분석은 다양한 임상연구 및 실제 임상에 활용할 수 있는 매력적인 방법이다. 체성분분석의 적용대상 및 상황에는 운동선수 또는성장 및 노화과정(근감소증)과 같은 생리학적 및 이상생리학적 상태뿐만 아니라 다양한 인구 및 질병(예: 비만, 섭식장애, 내분비계, 위장관계, 신장 및 감염성 질환)이 포함된다[2325].

2013 International Society for Clinical Densitometry (ISCD) 공식 입장 가이드라인은 체성분분석에서 DEXA 의 보다 실질적인 사용을 승인하고 공개했으며, 특히 세 가지 병리학적 상태에서 전신 DEXA 검사의 활용을 제안했다.

  • • HIV 양성 환자에서 지방위축(lipoatrophy) 위험과 관련한 항레트로바이러스제 사용 시 지방분포 평가

  • • 비만수술(또는 상당한 체중감소가 예상되는 약물, 식단 또는 체중감소요법) 시행 환자에서 체중감소가 10%를 초과할 때 지방과 제지방량을 평가

  • • 근력이 저하되거나 신체기능이 좋지 않은 환자의 지방 및 제지방량을 평가

ISCD 공식 가이드라인에 따라 골밀도, 골무기질함량, 총 질량, 총 제지방량, 총 지방량 및 체지방률은 체성분분석 연구와 보고서에 기본 변수로 나타내야 한다[26,27].

최근 몇 년 동안 근육량과 비만도에 대해 보다 구체적으로 평가하는 DEXA 지표와 측정이 확립되었지만, 아직 임상적 유용성은 불확실한 상황이다. 지방량과 근육량의 DEXA 측정지표에는 체지방지수(fat mass index [FMI] =total fat mass/height2), 내장지방조직(visceral adipose tissue, VAT), 피하지방조직(subcutaneous adipose tissue, SAT), 남성형(android, A) 대 여성형(gynoid, G) 지방량비(A/G 지방량비), 몸통 대 다리 지방량, 제지방량 지수(lean mass index: total lean mass/height2), 사지 제지방량(appen-dicular lean mass [ALM] =arms LM + legs LM) 및 사지근육량을 키의 제곱으로 보정한 ALM/height2, 체중으로 보정한 ALM/weight과 BMI로 보정한 ALM/BMI 등이 있다[27,28]. 제지방지수, ALM/height2, ALM/weight 및 ALM/ BMI 등은 근감소증에서 ‘low muscle mass’의 진단지표로 사용될 수 있다. 2010년에 유럽노인근감소증진단그룹(European Working Group on Sarcopenia in Older People, EWGSOP)에서 근감소증을 근육량과 더불어 근력이나 신체수행능력이 점진적이고 전반적으로 감소한 상태로 정의한 이래로 근감소증을 진단하기 위해서는 근육량뿐만 아니라 근력이나 신체수행능력도 평가해야 한다[29]. ALM/height2은 신체장애와 사망위험의 좋은 예측인자로 밝혀지면서, 최근의 2018년 EWGSOP의 개정안과 2019년 아시아근감소증진단그룹(Asian Working Group for Sarcopenia, AWGS)의 지침에서 사지근육량을 신장의 제곱 값으로 나눈 것을 근육량 감소의 주 지표로 권고하고 있다[30,31]. 사지근육량을 BMI나 체중으로 보정한 지표값이 ALM/height2보다 대사적 지표들과 관련성이 높다거나 근육기능감퇴와 장애를 예측하는 데 더 낫다는 보고들이 있으나[32,33], 이에 대해서는 연구가 더 필요하다. 최근의 DEXA 프로그램은 별도의 근력과보행속도를 측정해서 입력하면 근육량을 평가해서 근감소증 여부를 평가해주고 있다(Fig. 3).

비만지표에 있어서는 BMI와 상관 없이 총 지방량, FMI, 체지방률 또는 총지방/근육비는 심혈관대사질환 발병위험 증가와 관련이 높다고 밝혀졌다[34]. 또한, 인체의 지방분포가 총 지방량보다 임상적으로 더 중요할 수 있다고 알려졌다[35,36]. 이러한 이유로, 지방량의 중추 및 말초분포를 나타내는 DEXA 변수들인 남성형(A)과 여성형(G) 체지방량과 A/ G 지방량비 및 몸통/다리 지방량비, 그리고 VAT, SAT 및 VAT/SAT 등이 임상에서 유용하게 사용될 수 있다. DEXA 기기는 A/G 지방량비를 남성형과 여성형 측정 부위(region of interest, ROI) 사이의 지방량 비율로 제공하는데, 남성형 ROI는 일반적으로 마지막 늑골과 장골능선 사이의 영역으로 정의한다. 여성형 ROI는 남성형 ROI 높이의 1.5배 아래에 상부 경계선이 위치하며, 대퇴둔부 부위를 포함하여 하부 경계선은 상부 경계선으로부터 남성형 ROI 높이의 2배 길이만큼 아래를 통과하는 선에서 결정한다[37]. 허리둘레, 허리-엉덩이둘레 비율을 각각 대표하는 DEXA 변수인 남성형 체지방량과 A/G 지방량비는 인슐린저항성뿐 아니라 이상지질혈증과도 연관성이 높다[38]. 몸통/다리 지방량비는 치료과정에서 지방위축증이나 지방이상증이 발생하는 환자의 지방의 재분포를 평가할 때 지표로 활용할 수 있다[27]. FMI는 초과 체중이 아닌 초과 지방량을 기반으로 한 새로운 비만분류 시도에 BMI의 대체지표로 시도되어 왔다[27]. FMI는 성별 및 인종별 참조 범위를 사용하며, 아직 임상적 유용성은 확립되지 않았지만 근육량이 많은 사람에서 비만을 평가할 때 BMI 보다 더 유용할 수 있을 것이다. 비만과 관련한 여러 학회 및기관들에서도 근육량이 많은 개인을 평가할 때 BMI만으로 비만을 평가하지 않도록 권고하고 있으며, 체성분분석이 비만 수준을 결정하는 데 도움이 된다고 제시하고 있다[27,39].

최근 전신 DEXA 기술의 발전 중 하나는 VAT 평가가 가능하다는 것이다(Fig. 4). 최신의 전신 DEXA 프로그램은 먼저 남성형 ROI에서 복강 내외의 지방을 감지하여 SAT의 양을 추정한다. 그런 다음 이 SAT 추정치를 남성형 ROI의 총 지방량에서 빼서 최종 VAT의 양이 계산된다. DEXA VAT 진단기준과 관련하여 일반적으로 CT에서 VAT는 전통적으로 cm2 단위로 표현하며, DEXA에서는 VAT의 부피, 무게, 단면적 모두를 제공해 준다. 또한 DEXA-VAT 측정치는 CT로 얻은 수치와 높은 상관관계가 있는 것으로 나타났다(r =0.93) [40]. DEXA-VAT 측정은 CT에 비해 몇 가지 장점이 있으며, 낮은 방사선양에도 측정값의 정확도가 개선된 점이 가장 큰 장점일 것이다. VAT의 임상적인 중요성이 알려지면서 여러 연구들에서 VAT가 SAT 또는 A/G 지방량비보다 사망률과 심혈관질환 위험을 더 잘 예측하는 것으로 나타나고 있다[27,41]. 정상 BMI라 하더라도 내장비만 정도가 높을 수 있으므로, 기존의 신체계측법만으로는 마른 비만(metabolically obese normal weight)을 감별하기가 어려울 수 있다. 특히, 노인 또는 만성질환을 가진 환자에서 근감소증이나 비만도를 평가시에 근육량, 지방량 및 내장지방을 동시에 측정하는 DEXA 검사를 통해 진단의 오류를 줄일 수 있을 것이다.

DEXA를 이용해서 측정한 ALM (DEXA-ALM)은 low muscle mass의 중요한 지표이고, CT로 측정한 VAT (CT-VAT)는 내장비만평가기준으로 활용해 왔다. 근감소증과 내장비만은 서로 대사적으로도 악영향을 끼쳐 대사질환과 죽상동맥경화증에 상당한 부정적인 영향을 미칠 수 있지만, 지금까지 대부분의 연구는 DEXA-ALM과 CT-VAT를 이용해서 근감소증과 내장비만의 역할을 별도로 조사하거나 각각의 장비로 측정한 DEXA-ASM과 CT-VAT의 비율을 근감소성 비만지표로 활용해 왔다[9,42]. 최신의 DEXA 장비로 사지근육량뿐만 아니라 VAT를 동시에 측정할 수 있고 편리성과 다양한 체성분분석결과를 제공함으로써 DEXA의 적용 범위는 지속적으로 확대될 것으로 기대된다.

결론

DEXA를 이용한 체성분분석은 정확도에 있어 CT와 MRI 와 근접한 수준이고 다른 검사장비들과 비교하여도 여러 장점이 있어 활용이 증가하고 있다. 전신 DEXA는 과거에 체지방률을 평가할 때 제한적으로 사용한 경험이 있었고, 최근의 기술은 부위별 체성분분석의 정확도가 향상되어 복부의 VAT와 SAT를 구별하여 평가하고 있다. 무엇보다 2019년에는 근감소증을 진단하고 치료효과를 모니터링하는 데 신의료기술로 인정받아 근감소증 환자 및 의심 환자를 대상으로 근육량을 평가하는 데 활용되고 있다. 또한 DEXA는 CT 와 MRI에 비해 장비 가격이나 검사비가 낮고 덜 침습적이며, 무엇보다도 낮은 방사선양에도 불구하고 비슷한 정확도를 제공하고 있다. 노인에 있어 쇠약(frailty), 근감소증 또는 비만이 의심되는 경우 진단 및 중재 효과 판정 등 여러 목적으로 DEXA를 활용할 수 있겠고, 고령이 아니더라도 만성질환으로 인한 신체구성의 변화를 모니터링하거나 예후를 평가하는 데도 도움을 줄 수 있을 것이다. DEXA를 이용한 신체구성요소와 기존의 임상지표들(예: 내분비, 대사 및 염증성 지표)과 결합한 연구들과 그 결과들은 향후 근감소성 비만의 정의와 진단기준 확립에 중요한 기초자료가 될 것이다.
