Diabetic Cardiac Autonomic Neuropathy: Symptoms, Diagnosis and Treatment
Ji Hyun Lee
Abstract
Cardiovascular autonomic neuropathy (CAN), an independent predictor of cardiovascular disease, is the most studied and clinically important form of diabetic autonomic neuropathy. Conclusive clinical evidence shows that hyperglycemia plays a central role in the pathogenesis of CAN. CAN may be indicated by resting tachycardia (> 100 bpm), orthostasis (a fall in systolic blood pressure > 20 mmHg upon standing) or other disturbances in autonomic nervous system function involving the gastrointestinal and genitourinary systems. As with diabetic peripheral neuropathy treatments, these interventions do not change the underlying pathology and natural history of the disease process but may have a positive impact on the quality of life of the patient.
