A Case of Type 1 Diabetes Mellitus Occurred During Interferon-α Therapy for Chronic Hepatitis C

Abstract
We report a case of 58-year-old woman with chronic hepatitis C treated with interferon (INF)-α and ribavirin. Twelve months after the start of treatment, the patient developed marked polydipsia, polyuria and severe weight loss. At that time random plasma glucose level was 600 mg/dL and plasma C-peptide level was 0.10 ng/mL. The titer of anti-GAD antibody was 60.9 u/mL and anti-islet cell antibody was positive (Scimedx, U.S.A.). Type 1 diabetes mellitus was diagnosed and administration of INF-α and ribavirin was withdrawn and insulin treatment was started. After 2 months of insulin therapy, the titer of plasma C-peptide level was 0.10 ng/mL and GAD antibody was 65 u/mL. Screening for autoantibodies against pancreatic β-cell is recommanded before and during interferon therapy to identify subjects at high risk of developing type 1 diabetes mellitus.
