당뇨병신경병증의 신경염증 기전과 새로운 생물표지자

Abstract

Diabetic neuropathy (DN) is one of the most common and debilitating complications of diabetes, and its prevalence is expected to increase with the growing global diabetes burden. DN is regarded as a neurodegenerative disorder; however, a definitive disease-modifying therapy remains unavailable. Therefore, early recognition and appropriate management are crucial to prevent irreversible nerve damage and to improve clinical outcomes. Despite advances in diagnostic techniques, the early detection of DN remains challenging. Research has been devoted to identifying novel biomarkers that reflect the complex pathophysiological mechanisms underlying DN. Following the early metabolic disturbances triggered by nutritional excess, multiple pathogenic pathways become activated, leading to chronic lowgrade inflammation. Numerous studies have explored inflammatory biomarkers, including cytokines, chemokines, and immune receptors, which play key roles in the development and progression of DN. Furthermore, growing evidence highlights the pivotal involvement of immune cells mediating neuro- immune crosstalk, supported by molecular data elucidating the underlying mechanisms. This review summarizes recent advances in the discovery of novel biomarkers for early diagnosis of DN, with a particular emphasis on neuroinflammation-related pathways.

서론

당뇨병신경병증(diabetic neuropathy)은 가장 흔한 당뇨병합병증의 하나로, 당뇨병의 증가 추세에 따라 발생빈도가 증가할 것으로 예측된다[1,2]. 당뇨병신경병증은 다양한 임상 양상을 나타낼 수 있고, 삶의 질 저하 및 일상생활의 제한을 동반하며, 발궤양(foot ulcer) 및 절단(amputation)의 위험을 높이고, 이는 궁극적으로 사망률 증가와 관련된다[3,4]. 하지만 임상적 중요성에도 불구하고 당뇨병신경병증은 다른 당뇨병합병증(당뇨병신장병증 또는 당뇨병망막병증)에 비해 상대적으로 적은 관심을 받는 경향이 있다. 당뇨병신경병증은 신경퇴행성 질환(neurodegenerative disorder)으로 길이 의존성 신경병증(length-dependent neuropathy)의 특성을 지니기 때문에 더 긴 감각신경이 우선적으로 손상되므로 말단 대칭성 다발신경병증(distal symmetric polyneuropathy)이 가장 흔하며, ‘stocking and sock’ 형태로 묘사되기도 한다[5]. 다만, 환자의 최대 50%까지도 증상이 없어 진단이 지연되거나 간과되기 쉽다[6]. 현재까지 질병의 진행을 근본적으로 변화시킬 수 있는 치료법이 없기 때문에 혈당을 적절히 조절하고, 통증을 포함한 신경병증의 증상을 완화하며, 미세혈관 순환을 향상시키는 데 치료의 중점을 두고 있다. 따라서 예후를 개선하기 위해서는 당뇨병신경병증의 조기 발견과 적절한 관리가 매우 중요하다.
당뇨병신경병증은 다른 가능한 원인을 배제한 후, 당뇨병 환자에서 말초신경 기능장애의 증상 및 징후가 존재하는 경우에 진단할 수 있으며, 병력 청취와 신경학적 검사를 통하여 주로 진단된다[7]. 앞서 언급한 바와 같이, 감각신경이 운동신경보다 더 빠르고 심하게 손상되므로 감각기능 검사가 진단에 있어 중요한 역할을 한다. 검사방법으로는 MNSIQ (Michigan Neuropathy Screening Instrument Questionnaire)를 포함한 설문 평가와 신경학적 검사(10 g 모노필라멘트 검사, 진동감각 검사, 발목반사 검사, Pinprick 검사, 온도감각 검사 등)를 시행할 수 있다[8]. 여러 임상적 평가지표가 사용되고 있지만 당뇨병신경병증의 징후와 증상은 불명확한 경우가 많아 증상만으로는 조기 진단에 어려움이 존재한다. 더불어, 현재의 선별검사들은 주로 대신경섬유(large fiber) 기능 이상에 대한 주관적 평가에 의존한다는 제한점이 있다. 임상 양상이 비전형적인 경우, 보다 정확한 검사법으로 신경전도검사(nerve conduction study) 및 피부 생검을 통한 표피 내 신경섬유 밀도(intraepidermal nerve fiber density) 측정을 고려할 수 있으나, 이는 침습적이며 숙련된 검사자와 고가의 장비가 요구된다[9]. 최근에는 미세신경섬유의 기능 및 구조적 변화를 평가하기 위한 각막공초점현미경(corneal confocal microscopy)이 상대적으로 높은 진단율을 보여주고 있으나, 현재까지는 명확한 기준치가 제시되고 있지 않아 이에 대한 추가 연구가 필요하다[10,11].
현재 사용되는 검사법들의 한계를 벗어나기 위해 당뇨병신경병증의 병태기전을 이용하여 새로운 생물표지자(biomarker) 및 치료법을 개발하려는 노력들이 진행되고 있으나, 복잡한 병태생리학적 기전과 일부 완전히 규명되지 않는 부분들로 인하여 어려움을 겪고 있다[12]. 에너지 기질 과부하(energy substrate overload)에 의해 유발되는 신경 대사이상과 생체에너지 실패(bioenergetic failure)가 병태생리의 중심이다[12]. 이러한 대사 불균형은 다양한 하위 대사 경로를 활성화시킴으로써 염증유발사이토카인(proinflammatory cytokines)의 분비, 미토콘드리아 기능장애, 세포질세망스트레스(endoplasmic reticulum stress)의 촉진, 최종당화산물(advanced glycation end-products)의 축적, 반응산소종(reactive oxygen species) 생성 등을 유발하고, 면역세포의 활성화를 유도하여 만성적인 저등급 염증(lowgrade inflammation)을 초래하며, 궁극적으로 신경독성 손상(neurotoxic injury)으로 이어진다[13]. 따라서 이러한 비가역적 신경 손상이 발생하기 전에 조기 발견과 적절한 중재가 필수적이다. 이에 본 시론에서는 신경염증을 중심으로 당뇨병신경병증의 새로운 생물표지자와 관련된 최신 연구 동향을 알아보고자 한다.

본론

1. 대사성 생물표지자

대사 조절 이상은 당뇨병합병증의 발생과 진행 과정 모두에서 핵심적인 역할을 하며, 진단적 및 예측적 가치를 지닐 수 있다. 27개의 연구에서 관찰되는 21개의 예측 인자들을 메타분석한 연구에서는 연령, 당화혈색소, 체질량지수, 당뇨병 유병기간 및 수축기혈압이 당뇨병신경병증의 발생과 강한 연관성이 있음을 보고하였다[14]. 멘델 무작위화(Mendelian randomization) 분석에서 당뇨병신경병증은 총지방산 대비 DHA (docosahexaenoic acid) 비율의 감소, 매우 큰 초저밀도지단백질(very low density lipoprotein) 입자 내 중성지방 대 총지질의 비율 증가 및 혈중 파이루브산염(pyruvate) 농도 상승과 상관성을 보여주었다[15]. 총 479명의 참가자를 세 그룹(정상 대조군 및 당뇨병신경병증 유무에 따른 2형당뇨병 환자)으로 나누어 혈청 내 20종의 아미노산 농도를 측정하여 비교한 연구에서, 신경병증을 동반한 당뇨병군은 신경병증을 동반하지 않은 당뇨병군에 비해 유의하게 낮은 아르지닌(arginine) 및 타이로신(tyrosine) 농도와 높은 글루탐산(glutamic acid) 농도가 관찰되었다[16]. 미토콘드리아는 에너지 대사, 반응산소종 생성 및 세포자멸사(apoptosis) 등에 관여함이 잘 알려져 있기에[17] 이전의 많은 연구들은 미토콘드리아의 구조 및 기능적 이상에 초점을 맞추었으며, 최근에는 융합(fusion)과 분열(fission) 간의 미토콘드리아 역동성(mitochondrial dynamics) 조절에 관심이 집중되고 있다[13,17]. 그러나 이러한 기전적 이해에도 불구하고, 이를 반영할 수 있는 신뢰할 만한 생물표지자의 개발은 여전히 과제로 남아 있다.

2. 신경염증과 관련된 생물표지자

과잉영양 상태가 초기 유발 인자로 작용한 이후, 여러 손상 기전이 활성화되면서 장기적인 염증상태가 유도되며, 이 과정에서 TNF-α (tumor necrosis factor-alpha), IL-6 (interleukin-6), IL-1β (interleukin-1 beta), NF-κ B (nuclear factor kappa-light-chain-enhancer of activated B cells), TLR-4 (Toll-like receptor 4), TGF-β (transforming growth factor-beta), CXCL (C-X-C motif chemokine ligand), MCP-1 (monocyte chemoattractant protein-1), C-reactive protein (CRP) 등의 염증유발사이토카인은 유의하게 증가하고, 반대로 BDNF (brain-derived neurotrophic factor) 및 NGF (nerve growth factor)와 같은 신경영양인자(neurotrophic factors)는 감소한다[18-20]. 이러한 인자들은 신경 염증과 관련된 잠재적 생물표지자로 연구되고 있다[21]. 당뇨병신경병증과 관련하여 염증성 표지자에 대한 수많은 연구가 진행되어 왔으며, 여기에는 사이토카인(cytokines), 케모카인(chemokines), 그리고 면역 수용체(immune receptors)가 포함되었다. 최근에는 혈액세포 유래 지표(blood cell-derived indices)와 관련한 연구들이 보고되고 있으며[22], 호중구/림프구 비율(neutrophil-to-lymphocyte ratio, NLR)이 대조군에 비해 당뇨병신경병증군에서 유의하게 높았으나, IL-1Ra (interleukin-1 receptor antagonist)와 고감도 CRP (high sensitivity CRP)는 유의한 차이를 보이지 않았다[23]. NLR과 PLR (platelet-to-lymphocyte ratio)과 같은 혈액세포 유래 지표는 다른 생물표지자들에 비해 흔하게 시행되는 채혈검사를 통해 시행할 수 있어 접근성이 높고 비용 효율적이라는 장점이 존재한다. 하지만 대부분이 단면연구에 기반하고 있어 인과적 해석에 한계가 있으며, 이러한 염증 관련 변화는 비만, 심혈관질환 및 신경병증 외의 당뇨병합병증 등에서 흔히 동반되는 만성염증과 중복되어 나타나므로, 신경병증에 특이적인 생물표지자를 규명하는 데 어려움이 있다.
면역세포는 선천적 및 후천적 염증반응을 조절함으로써 신경-면역 교차 작용(neuroimmune crosstalk)의 중심적인 역할을 한다. 대식세포(macrophage)에 관한 많은 연구들이 시행되었으나[24], 신경에는 손상 신호를 감지하고 미세환경의 항상성을 유지하기 위해 소수의 세포 유형(minority cell types)도 존재한다. 외상성 절단(traumatic limb amputation)과 당뇨병신경병증 환자들의 경골 신경조직을 비교했을 때, 신경병증에서는 염증반응, ERK1/2 경로 및 백혈구 화학주성(leukocyte chemotaxis)과 관련된 유전자 발현이 유의하게 증가되어 있었다[25]. 신경병증 환자의 경골신경 조직에서 더 많은 비만세포(mast cell)가 관찰되었고, 추가로 시행된 동물실험에서 성숙한 비만세포가 결핍된 당뇨병신경병증 쥐 모델에서는 신경전도속도가 더 빠르고, 축삭 밀도와 슈반세포(Schwann cell)의 수가 향상되는 결과를 보였다. 비만세포 활성화는 GLUT-3-ERK-mTOR 신호 경로를 통해 매개됨이 관찰되었다[25].

3. 분자유전학적(molecular and genetic) 생물표지자

당뇨병신경병증에 동반되는 신경변성의 기저 분자기전은 여전히 명확히 규명되지 않았다[26]. 당뇨병신경병증 환자군과 대조군 간의 유전자 발현 차이를 혈액과 조직에서 분석했을 때, 발현이 증가된 유전자는 주로 면역계 및 심혈관계 질환과 관련된 것으로 나타났다. 두 개의 Gene Expression Omnibus (GEO) 데이터세트를 기반으로 분석한 결과, 면역 관련 유전자 세 가지(Fc epsilon receptor Ig, FCER1G; protein tyrosine kinase 2 beta, PTK2B; spleen associated tyrosine kinase, SYK)가 당뇨병신경병증과 연관되어 있었으며, 이들 유전자가 당뇨병신경병증의 잠재적 진단 및 예후 예측 생물표지자로 활용될 가능성을 보였다[27]. 1형당뇨병 환자의 말초혈액 단핵세포(peripheral blood mononuclear cells)에서 당뇨병신경병증의 유무에 따른 long noncoding RNA를 비교 분석하였을 때, MALAT1 (metastasis associated lung adenocarcinoma transcript 1), NEAT1 (nuclear enriched abundant transcript 1), XIST (X-inactive specific transcript), DLX6-AS (distal-less homeobox 6 antisense) 유전자는 당뇨병신경병증 환자군에서 유의하게 증가하였으며, MEG3 (maternally expressed gene 3) 유전자는 감소되는 것으로 나타났다[28]. MALAT1은 산화 스트레스와 미토콘드리아 기능장애를 유발하고, NEAT1은 신경세포 사멸(neuronal apoptosis)을 촉진하고 신경생성을 저해하는 것으로 알려져 있다. 반면, MEG3의 발현 감소는 세포사멸 조절의 손상과 연관되어 있어 신경세포의 취약성을 증가시킨다[28]. 이러한 새로운 유전체 발현지표들이 당뇨병신경병증의 조기 발견 및 위험 모니터링을 위한 예측적 생물표지자로 활용될 가능성이 있다.

4. 신경세포 손상과 관련된 생물표지자

Neuron-specific enolase (NSE)는 신경세포에 특이적인 고용해성 당분해효소(glycolytic enzyme)로, 당뇨병환자에서 신경병증이 있는 경우 그렇지 않은 환자에 비해 NSE 수치가 유의하게 높게 나타남에 따라 신경손상의 잠재적 생물표지자로 주목을 받았으나[29] 임상적으로 상용화되지는 못하였다. 기술의 발전으로 말초혈액에서 신경세포의 구조와 관련된 특이 단백질을 측정할 수 있게 되었다[30]. 이들 중에서 neurofilament light chain (NfL)은 대섬유 신경병증(large-fiber neuropathy)을 탐지하는 데 가장 적합한 표지자로 알려져 있으며, glial fibrillary acidic protein (GFAP)은 소섬유 신경병증(small-fiber neuropathy)의 표지자 후보 물질로 여겨지고 있다[31]. 덴마크에서 시행된 DIAFALL 코호트의 바이오뱅크 데이터를 이용한 연구에서는 당뇨병신경병증 환자의 혈청 GFAP 수치가 신경병증이 없는 당뇨병환자 및 건강 대조군에 비해 유의하게 낮은 것으로 나타났다[32]. 하지만 GFAP는 혈관질환 등 당뇨병환자에게 흔한 급성 중추신경계 손상에서도 민감하게 증가하는 생물표지자이기 때문에, GFAP와 NfL을 종적(longitudinal)으로 비교하고 시간 의존적 차이를 탐색하는 연구를 통해 두 표지자의 병용 사용 가능성을 알아보는 등의 추가적인 연구가 필요하다.
철사멸(ferroptosis)은 최근에 규명된 새로운 형태의 세포사(cell death) 기전 중 하나로, 세포 내 제2철(Fe2+) 이온의 증가와 지질과산화를 통해 생성된 반응산소종의 축적이 특징적이다[33]. Arachidonate 15-lipoxygenase (ALOX15)는 이 과정에 관여하는 주된 효소로, 아라키돈산(arachidonic acid)이나 리놀레산(linoleic acid)과 같은 고도불포화지방산(polyunsaturated fatty acids)의 과산화를 촉매하여 철사멸을 유도한다. 당뇨병신경병증군에서는 ALOX15, malondialdehyde (MDA), 4-Hydroxynonenal (4-HNE) 수치가 대조군에 비해 유의하게 증가하였으며, ferritin heavy chain 1 (FTH1) 발현은 현저히 감소하였다. MDA와 4-HNE는 지질과산화 과정에서 생성되는 알데하이드 형태의 독성 부산물로 세포에 세포독성 효과를 유발한다. 반면, FTH1은 정상적으로는 세포 내 철 저장에 관여하지만 그 발현이 감소하면 세포 내 제2철 농도가 상승하여 철사멸을 더욱 촉진시킨다[34]. 이에 ALOX15가 잠재적 신경손상의 잠재적 생물표지자로 제안되었다.

5. 기타 생물표지자

웨어러블 기기는 운동, 자율신경, 미세혈관 기능 등 다양한 영역에서 연속된 데이터를 제공할 수 있으므로, 이를 이용하여 당뇨병신경병증을 조기에 진단하기 위한 연구들이 진행되고 있다. 자세 흔들림(postural sway)은 정적 또는 동적 자세 유지 중 신체의 압력 또는 질량 중심의 이동을 시간에 따라 정량화한 운동학적 지표로, 균형 유지 능력을 반영하므로[35] 흔들림 속도의 증가는 자세 안정성 저하를 의미하며, 이는 웨어러블 센서를 이용해 측정이 가능하다. 당뇨병신경병증 환자에서는 자세 흔들림 속도와 총 이동거리가 모두 증가함이 관찰되었고, 균형장애와 관련된 인자들이 신경병증의 임상적 중증도 및 신경병증의 전기생리학적 지표와 밀접하게 연관되어 있음을 시사하였다[36]. 또한 연속 모니터링과 실시간 데이터를 통하여 족저 압력, 피부 온도 및 습도, 산소포화도, pH 등을 측정하여 발한기능(자율신경) 분석 및 상처의 3D 계측 등을 시행할 수 있다[37].

결론

당뇨병신경병증의 치료 경과를 바꿀 수 있는 치료법이 개발되어 있지 않으므로, 조기 진단을 통한 적절한 관리가 예후 향상에 중요하다. 비가역적인 신경 손상이 발생하기 전 단계에서 발견하기 위한 많은 연구들이 진행되었으나, 현재까지 사용 가능한 검사법들은 조기 진단에 많은 제한점을 보여왔다. 당뇨병신경병증의 병리생태 기전에서 대사이상과 만성염증이 주요 기전으로 작용함이 잘 알려지고, 특히 신경염증 기전의 중요성이 대두되면서 이와 관련된 기전을 이용한 생물표지자들이 연구되고 있다. 현재까지는 일상 임상에 적용하기에 여러 한계가 존재하여, 추후 비침습적이고 접근성이 높으며 고감도의 새로운 생물표지자 개발을 통해 당뇨병신경병증의 조기 진단 및 중재를 가능하게 하는 것이 필요하다.
