Dipeptidyl Peptidase 4 억제제 최신지견

Abstract

Dipeptidyl peptidase 4 (DPP4) inhibitors, one of the most prescribed second-line antidiabetic drugs in South Korea, have been available since 2006. Many studies focusing on the safety profiles of DPP4 inhibitors have been reported. DPP4 inhibitors were associated with a small increased incidence of acute pancreatitis in placebo-controlled trials, although most observational studies did not support this hypothesis. There were no significant differences in major cardiovascular events in a large prospective cardiovascular outcomes trial with DPP4 inhibitors, although a higher risk of hospitalization for heart failure was reported with saxagliptin. Additionally, the safety data of DPP4 inhibitors for diabetes management during the coronavirus disease-19 (COVID-19) pandemic have been provided recently.

서론

경구로 포도당을 섭취하게 되면 정맥혈에 포도당을 주입했을 때보다 췌장에서의 인슐린 분비가 훨씬 더 높다는 것 이 알려지고[1] 이러한 인크레틴 효과가 위장관 분비 호르몬인 글루카곤유사인슐린자극펩타이드(glucose-dependent insulinotropic polypeptide, GIP)와 글루카곤유사펩타이드(glucagon-like peptide 1, GLP-1)에 의한 것이 밝혀지면서 [2,3] 이를 이용한 당뇨병 치료 약제 개발이 시작되었다. 분화무리(cluster of differentiation)-26이라고도 불리는 다이펩타이드분해효소(dipeptidyl peptidase 4, DPP4)는 혈중에 유리 형태로 존재하거나 혹은 위장관 점막 등 대부분의 조직 내피 및 상피세포에서 발현되는 효소로 생리적 기능을 가진 다양한 펩타이드의 질소 말단을 분해한다[4]. 인크레틴 호르몬인 GIP와 GLP-1은 DPP4의 주요 표적으로 DPP4에 의해 수분 만에 분해되어 비활성화된다. 이를 극복하고자 개발된 DPP4 억제제는 GLP-1과 GIP의 분해를 억제하여 내인성 인크레틴 효과를 증가시킨다.
2006년도에 sitagliptin이 미국식품의약국(U.S. Food and Drug Administration, FDA)에서 승인된 후 우리나라에서는 2021년 현재 총 9개의 DPP4 억제제가 처방이 가능하다. DPP4 억제제와 메트포민과의 복합제도 많이 출시되었으며, 우리나라에서 가장 흔히 사용되는 당뇨병의 2차 약제로 보고된다. 최근까지 DPP4 억제제와 관련된 심혈관질환 안전성에 대한 대규모 연구도 활발히 이루어지고 있다. 본 글에서는 출시 초기에 제기되었던 여러 부작용에 대한 메타분석 및 심혈관계 안정성에 대한 연구 결과와 함께, 2021년 현재 전 세계적 건강 위기를 일으킨 COVID-19의 범유행과 관련된 DPP4 억제제 사용의 안전성 연구 결과를 요약해보고자 한다.

본론

1. 췌장 질환 안전성

DPP4 억제제는 약제의 기전상 GLP-1을 높이기 때문에 외분비 췌장에 대한 부작용, 즉 췌장염 및 췌장암의 발생에 대한 우려가 존재한다. GLP-1 수용체는 췌도와 췌관에 발현을 하고, 이론적으로 지속적인 GLP-1의 과자극은 췌관의 증식과 이형성 및 췌관 폐색을 일으켜서 췌장염을 일으킬 수 있다[5]. 미국 FDA와 유럽 의약청(European Medicine Agency)은 인크레틴에 기반한 약제들을 사용한 환자에게서 시판 후 췌장염과 췌장암 보고를 통한 안전성 평가를 시행하였다. 두 기관은 2014년도에 DPP4 억제제와 췌장염 및 췌장암과의 인과관계는 일관성이 없다는 데에 동의하였으나 지속적인 췌장 안전성에 대해서 조사해야 한다고 결론지었다[6].
그 이후 발표된 3개의 심혈관계 영향 연구(SAVOR-TIMI 53: saxagliptin, EXAMINE: alogliptin, TECOS: sitagliptin)를 포함한 36개의 무작위 대조군 연구를 대상으로 진행된 메타분석에서는 DPP4 억제제 사용이 급성 췌장염의 위험도를 증가시키지만(상대 위험도 1.57, 95% 신뢰구간 1.03~2.39), 췌장암의 위험도는 증가시키지 않았다[7]. 심혈관계 영향 연구 5개를 대상으로 진행된 메타분석에서도 DPP4 억제제 사용이 위약 사용군에 비하여 유의하게 급성 췌장염의 발생 빈도를 증가시켰다(승산비 1.81, 95% 신뢰구간 1.21~2.79, P = 0.04) [8]. 반면 캐나다, 미국, 영국의 7개 기관에서 건강 기록을 바탕으로 약 1천 500만 명의 2형당뇨병 환자 중 급성 췌장염으로 인한 입원을 분석한 대규모 국제 다기관 인구 기반 코호트연구에서는 DPP4 억제제 투약이 다른 경구혈당강하제 투약에 비하여 급성 췌장염 위험 증가와 관련이 없었다(보정 위험비 1.09, 95% 신뢰구간 0.86~1.22) [9]. DPP4 억제제를 24주 이상 투약한 165개의 임상연구를 대상으로 한 메타분석에서도 DPP4 억제제가 췌장염 및 췌장암의 위험도 증가와 관련이 없었다(Mantel-Haenszel 승산비 1.13, 95% 신뢰구간 0.86~1.47; Mantel-Haenszel 승산비 0.86, 95% 신뢰구간 0.60~1.24) [10]. DPP4 억제제와 췌장염과의 인과 관계는 아직까지 결론 지을 수 없으며 지속적인 감시와 추적 관찰이 필요하다.

2. 심혈관질환 안전성

심혈관질환은 2형당뇨병 환자에게서 이환율과 사망률을 결정하는 가장 중요한 인자이다. 따라서 당뇨병 약제는 심혈관질환 위험도에 대한 효과 및 안전성 결과가 매우 중요하다[11]. DPP4 억제제의 경우 현재까지 5개의 대규모 심혈관계 영향 연구가 이루어졌다(Table 1) [12-16]. 5개의 연구 모두 DPP4 억제제는 주요 심혈관질환 사건을 증가시키지도 감소시키지도 않았다. 그러나 saxagliptin을 대상으로 한 SAVOR-TIMI 53 연구에서는 심부전으로 인한 입원을 유의하게 증가시켰다[12]. 이에 따라 미국 FDA는 2016년도에 saxaglpitin과 alogliptin 투약이 심부전 위험을 증가시킬 수 있다고 경고하였다.
4개의 무작위 대조군 연구와 3개의 관찰 코호트연구를 대상으로 한 메타분석에서는 DPP4 억제제 사용이 주요 심혈관질환 사건을 증가시키지 않았고(승산비 0.95, 95% 신뢰구간 0.86~1.04) 심부전으로 인한 입원도 대조군과 차이가 없었다(승산비 1.05, 95% 신뢰구간 0.90~1.23) [17]. 한편 이 메타분석에서는 최근에 발표된 CARMELIA 및 CAROLINA 연구 결과는 반영되지 않았다. DPP4 억제제의 심부전에 대한 영향에 대해서는 이견이 많으며, 심부전 위험이 있는 2형당뇨병 환자에게서는 DPP4 억제제 사용 시 주의가 필요하다.

3. 기타 안전성

2015년도 미국 FDA에서는 DPP4 억제제 투약과 관련된 관절통 사례가 확인됨에 따라 DPP4 억제제가 관절통을 일으킬 수 있다고 권고하였다. 이후 발표된 67개 무작위 대조군 연구를 이용한 메타분석에서는 DPP4 억제제가 관절통 위험 증가와 관련이 있으나(상대 위험도 1.13, 95% 신뢰구간 1.04~1.22, P = 0.003), 심각한 관절통은 유의한 차이가 없다고 발표하였다(상대 위험도 1.44, 95% 신뢰구간 0.83~2.51, P = 0.20) [18]. 관절통의 발생 기전으로는 DPP4 억제제가 사이토카인을 증가시켜 이에 의한 염증 반응으로 발생한다고 여겨지고 있으나 정확한 기전은 연구가 더 필요하다.
DPP4 억제제 사용과 관련하여 물집유사물집증(bullous pemphigoid, BP)에 대한 사례가 보고되었고, 프랑스 약물감시 데이터베이스 분석에서도 DPP4 억제제, 특히 vildagliptin 사용 시 BP의 위험이 증가한다고 보고하였다(보고 승산비[reporting odds ratio] 67.5, 95% 신뢰구간 57.1~96.9) [19]. 2012년도부터 2016년도까지 우리나라 보험 청구 데이터를 활용한 사례 대조군 연구에서도 DPP4 억제제 사용이 BP의 위험을 유의하게 증가시키며(보정 승산비 1.58, 95% 신뢰구간 1.25~2.00, P < 0.001), vildagliptin, sitagliptin, linagliptin의 순서대로 보정 승산비가 유의하게 높았다[20]. DPP4 억제제에 의한 BP 발생 기전은 명확하지 않으나, 당뇨병 환자에게서 BP 발생 시 DPP4 억제제와의 관련성을 고려해봐야 하겠다.

4. DPP4 억제제와 COVID-19

2020년 전 세계는 새로운 유형의 코로나바이러스인 중증급성호흡증후군 코로나바이러스-2 (severe acute respiratory syndrome coronavirus-2, SARS-CoV-2)에 의한 코로나바이러스감염증-19 (coronavirus disease-19, COVID-19)의 범유행을 맞게 되었다. 당뇨병 환자의 COVID-19 감염률은 일반 인구와 유사하나, COVID-19 감염 시 중환자실 이용 등 증상 악화 및 합병증 발생 위험이 증가한다[21,22]. 한편, DPP4는 위장관 조직뿐 아니라 다양한 조직의 면역세포, 내피세포 및 폐포세포에도 발현한다. 세계건강기구 약물유해반응 데이터베이스를 이용한 사례 대조군 연구에서는 DPP4 억제제 사용이 다른 당뇨병 약제에 비하여 상기도 감염의 보고가 증가한다고 하였으나[23], 최근에 발표된 임상 연구에서는 DPP4 억제제 투약이 지역사회획득폐렴 발생 및 호흡기계 감염을 증가시키지 않았다[24,25].
코로나바이러스의 침입에는 바이러스의 돌기(spike, S) 단백과 숙주 세포막 표면의 특정 수용체와의 상호작용이 필요하다. SARS-CoV는 안지오텐신전환효소(angiotensin-converting enzyme, ACE)-2를 진입 수용체로 주로 사용하나[26], 중동호흡기증후군(Middle East respiratory syndrome, MERS)-CoV는 인간 세포에서 DPP4에 결합하여 감염을 일으킨다[27]. 한편, SARS-CoV-2의 S 단백은 주로 ACE-2와 결합하여 바이러스 침입이 일어나지만, 이론적으로 DPP4와도 결합이 가능하다고 보고되었다[28,29]. 이에 따라 DPP4 억제제가 2형당뇨병 환자의 COVID-19 임상 경과에 어떠한 영향을 미칠 수 있을지 논의되고 있으며, 관련하여 임상 연구 결과가 발표되고 있다.
2020년 2월부터 5월까지 우리나라의 대구 소재 대학병원에 COVID-19 감염으로 입원했던 1,082명의 환자를 대상으로 한 다기관 관찰 연구에서 당뇨병 환자는 당뇨병이 없는 환자에 비하여 사망률 및 중환자실 이용이 증가하였으나, 당뇨병 환자에서 이전 DPP4 억제제 사용이 COVID-19 관련 사망률이나 중증도에 영향을 주지 않았다[22]. 한국건강보험심사평가원을 이용한 코호트연구에서는 2차 혹은 3차 당뇨병 약제를 사용했던 환자들 중에서 DPP4 억제제를 사용한 경우 다른 당뇨병 약제에 비하여 통계적인 유의성은 없었으나 사망률(보정 위험도 0.74, 95% 신뢰구간 0.43~1.26) 및 중증 발현(보정 위험도 0.83, 95% 신뢰구간 0.34~1.53)을 감소시키는 경향을 보였다[30]. 프랑스에서 COVID-19로 입원한 2,449명의 2형당뇨병 환자를 대상으로 진행된 CORONADO (Coronavirus SARS-CoV2 and Diabetes Outcomes) 연구에서는 입원 전 DPP4 억제제를 사용했던 596명의 환자는 DPP4 억제제를 사용하지 않은 환자에 비하여 입원 7일 이내 인공호흡기 사용 및 사망률에 차이가 없었다. 따라서 COVID-19 범유행기에 당뇨병 치료 약제로 DPP4 억제제 사용에 제한을 둘 필요는 없다. 한편, COVID-19에 감염된 당뇨병 환자에서 적절한 당뇨병 약제의 선택에 대해서는 앞으로의 연구 결과를 지켜봐야 하겠다.

결론

현재까지 연구 결과를 바탕으로 DPP4 억제제는 비교적 유의한 부작용이 적어서 당뇨병 환자에게 2차 약제로 안정성을 고려한 좋은 선택지가 될 것으로 예상된다. 한편, 아직 이견이 있는 부작용에 대해서는 지속적인 관심을 가지고 약물감시 데이터베이스와 임상 연구 결과를 확인해야 하겠다.
