당뇨병과 심부전

Abstract

Heart failure in patients with diabetes has long been considered a consequence of coronary artery disease. However, recent epidemiological evidence from patients with diabetes has shown a significantly increased prevalence of heart failure in patients without significant coronary artery stenosis, suggesting a separate entity involving diabetic complications. Therefore, heart failure in patients with diabetes is now considered as an independent disease entity, ‘diabetic heart’. The mechanism of ‘diabetic heart’ could be coronary artery disease and diabetic cardiomyopathy caused by advanced glycation end products and altered calcium handling, as well as changes to energy metabolism and oxidative stress in the myocardium of patients with diabetes. Recent cardiovascular outcome trials of anti-diabetic medications demonstrated the protective effects of certain drugs against heart failure in patients with and without diabetes. This review summarizes the relationship between diabetes and heart failure, and informs treatment planning for the prevention of heart failure in diabetic patients.

서론

당뇨병환자에서의 심부전은 관상동맥질환에 의한 end result로 오는 것으로 이해되어 왔으나, 최근 관상동맥질환이 없는 환자에서도 심부전의 위험도가 증가하는 것으로 알려지면서 diabetic heart에 대한 연구가 활발히 이루어지고 있다. 또한, 최근 발표된 Diabetes Fact Sheet-합병증 편에 의하면 당뇨병환자에서 허혈성심질환이나 허혈성뇌졸중은 줄어들고 있는 반면 심부전의 유병률은 지속적으로 증가하고 있어, 한국인 당뇨병환자에서도 심부전이 이제까지 간과되어 왔던 합병증으로 재조명되고 있다[1].
미국심장학회는 ejection fraction (EF)에 따라서 EF가 감소한 심부전과 EF가 유지된 심부전으로 나눈다[2]. 또한 심부전의 stage는 학회마다 차이를 보이는데, 미국심장학회에서는 구조적 심장질환 여부에 따라서 A에서 D까지 나누고 있으며, New York Heart Association Functional Classification 은 생활 속 기능 여부에 따라서 I에서 IV까지 나누고 있다.
당뇨병환자에서의 심부전은 1972년 Rubler 등[3]의 보고가 가장 처음으로, 사구체신염이 있었던 4명의 당뇨병환자를 보고하였는데 흉부 사진상 심장비대가 있으며 심근 병리부검 소견에서는 심근비대와 심한 섬유증 소견이 있었다. 전형적인 당뇨병성 심근병증(diabetic cardiomyopathy)은 “관상동맥질환이나 고혈압이 없는 당뇨병환자에서 심근의 구조적, 기능적 이상”으로 정의한다[4]. 그러나 아직까지 당뇨병환자에서의 심근병증에 대한 정확한 기전은 많이 알려져 있지 않고, 당뇨병성 심근병증이 실제로 존재하는 질병인지, 아니면 실체가 없는 개념적인 병인지에 대해서도 논란이 있다.
당뇨병환자에서 심부전의 기전은 단일기전으로 설명할 수 없으나, 심장의 포도당 대사 이상이 오고 주된 에너지원(fuel)이 상대적으로 효율적인 fuel인 포도당에서 효율성이 상대적으로 떨어지는 유리지방산으로 바뀌게 되는 것이 가장 큰 변화이다[5]. 이러한 fuel의 전환은 심장 수축력과 기능 저하를 가져오고 좌심실과 좌심방의 부전을 가져오게 된다. 최근 당뇨병 약제의 굵직한 심혈관 질환 outcome 연구들에서 sodium-glucose cotransporter-2 (SGLT2) inhibitor가 심부전 예방에 탁월한 효과가 있는 것으로 알려지고, 반면 thiazolidinedione (TZD)은 심부전의 위험도를 유의하게 증가시키는 것으로 알려진다[6].
본 시론에서는 당뇨병환자에서 심부전 병리기전을 알아보고, 당뇨병 약제의 심부전 악화 및 예방효과를 정리하며, 당뇨병환자에서 심부전을 예방하기 위한 방법에 대해서 알아보고자 한다.

당뇨병과 심부전의 관계: 역학

2019년 대한당뇨병학회 Diabetes Fact Sheet에 의하면 심근경색이나 뇌졸중 등의 혈관질환 유병률은 감소하지만 심부전 유병률은 지속적으로 증가하고 있어서, 한국인 당뇨병환자에서 심부전은 중요한 합병증임을 시사하고 있다[1]. UK Prospective Diabetes Study (UKPDS) 연구에서는 이미 혈당 증가와 심부전 위험도와의 상관성을 보고하였고, 당화혈색소 1% 증가에 따라 심부전 위험도가 16% 증가하는 것을 보고하였다[7]. 또한 Ongoing Telmisartan Alone and in Combination with Ramipril Global Endpoint Trial (ONTARGET) 연구와 Telmisartan Randomized Assessment Study in ACE Intolerant Subjects with Cardiovascular Disease (TRANSCEND) 연구의 31,546명의 참가자에서 기저 혈당상태에 따라서 심부전의 위험도를 분석하였는데, 정상혈당에서 당뇨병으로 증가할수록 심부전 위험도가 지속적으로 증가하였다[8]. 저자가 건강보험공단 데이터를 분석한 연구에서는 2009년에 국가검진을 받은 9,720,200명의 심부전이 없는 한국인을 2015년까지 분석한 결과, 정상혈당인 대상자와 비교할 때 전당뇨병이 기저에 있었던 대상자에서 심부전 위험도가 8% 증가하였고, 기저에 당뇨병이 있는 대상자에서는 심부전 위험도가 86% 증가하였다[9]. 이상의 연구에서 혈당 증가와 심부전 위험도의 유의한 상관성을 알 수 있다.
당뇨병환자에서 심부전 위험도가 증가한다는 연구들도 많이 있다. Enalapril의 심부전 예방효과를 본 연구인 Studies of Left Ventricular Dysfunction (SOLVD)-P 연구의 하위분석에서는 기저에 당뇨병이 있는 4,223명의 환자에서 향후 심부전 위험도를 분석하였는데, 당뇨병이 있는 사람에서 없는 사람보다 심부전 위험도가 1.3배, 심부전에 의한 입원 위험도는 1.75배가 증가하는 것으로 분석되었다[10]. 미국의 사보험 데이터베이스인 Kaiser Permanente North Western Division 데이터로 분석한 연구에 의하면 연령이 증가함에 따라서 심부전 위험도가 지속적으로 증가했는데, 당뇨병이 있는 사람이 없는 사람에 비해 심부전 유병률이 높았다[11]. 또한 당뇨병환자에서 혈당 조절과 약제 임상연구들에서는 당뇨병환자에서 심부전 유병률이 10∼28%까지 보고되었다[12].
당뇨병이 있는 심부전 환자에서 예후에 관한 연구도 보고되었다. Irbesartan의 심부전에 대한 효과를 본 I-Preserve 연구에서는 4,128명의 심부전이 있는 환자에서 4.1년간 추적관찰을 했을 때, 당뇨병환자에서 심혈관 사망 위험도와 심부전에 의한 입원 위험도는 당뇨병이 없는 환자보다 1.75배 높았고, 사망 위험도는 1.59배 높았다[13]. Candesartan 의 심부전에 대한 효과를 본 Candesartan in Heart Failure Assessment of Reduction in Mortality and morbidity (CHARM) 연구에서도 당뇨병이 있는 환자에서 심부전에 의한 입원 위험도가 높았고, EF가 유지되어도 당뇨병환자는 당뇨병이 없어도 EF가 떨어진 환자만큼의 심혈관 사망률이나 심부전에 의한 입원 위험도가 증가되어 있었다[14]. 또한 CHARM 연구의 하위분석에서는 당화혈색소 1% 증가에 따라서 심혈관 사망률과 심부전에 의한 입원 위험도가 1.25배 증가하는 것으로 분석되었다[15].
반대로, 심부전이 당뇨병 위험인자일 수 있다는 주장도 있다. 심부전 임상연구들에서 보고된 당뇨병 유병률은 21∼47%까지 보고되었는데, 상당히 높은 비중이다[12]. 또한 Matsue 등[16]에 의하면 당뇨병 병력이 없으면서 심부전으로 입원한 환자들 중, 검사해보면 1/3 정도가 전당뇨병 혹은 당뇨병을 가지고 있었다[16]. 또한 CHARM program과 Eplerenone in Mild Patients Hospitalization and Survival Study in Heart Failure (EMPHASIS-HF) 연구에서는 당뇨병이 기저에 없었던 심부전 환자에서 1,000인년(patient-years)당 당뇨병 발생이 28명과 21명으로 비슷한 연령대의 일반 인구보다 유의하게 높았다[17,18].
이상에서 정리해보면 당뇨병환자에서 심부전 유병률이 당뇨병이 없는 사람보다 높으며, 여러 인자를 보정하고도 심부전 위험도는 증가하여 확실한 당뇨병 합병증으로 생각된다. 또한 반대로 심부전 환자에서도 당뇨병 위험도가 증가하고 심부전이 당뇨병의 위험인자로 분석되어, 한 방향으로가 아니라 두 질환 간의 ‘bidirectional relationship’로도 생각해볼 수 있겠다[19].

당뇨병과 심부전의 관계: 병리기전

당뇨병이 심근병증에 직접적으로 미치는 영향은 크게 고혈당에 의한 영향과 인슐린저항성에 의한 영향으로 나눌 수 있겠다. 고혈당으로 advanced glycation end product (AGE)가 심근세포에 축적되고, 전염증성 사이토카인의 증가로 extracellular matrix remodeling이 일어나 섬유화가 생긴다. 또한 고혈당과 인슐린저항성으로 산화스트레스가 증가하여 심근 자멸이 생긴다. 또한 당뇨병으로 심근 fuel로 효율이 가장 높은 포도당보다 유리지방산을 쓰는 방향으로 에너지 대사가 바뀌어 에너지 효율이 낮아지고 이 역시 심근 변화를 유발하여 섬유화를 조장한다. 그 외에 인슐린저항성은 심근 내 칼슘 항상성 이상을 유발하여 심근 비후와 심장이완장애가 생긴다.
당뇨병이 심장에 미치는 구조적인 변화는 strong heart study의 결과에서 볼 수 있다[20]. 1,810명의 당뇨병환자와 944명의 정상인에서 분석하였는데, 당뇨병환자가 좌심실 질량과 심실벽이 더 크고 두꺼우며 좌심실의 수축기 chamber 크기가 작고 심근기능이 감소하며 동맥의 경화도가 증가되어 있었다[20]. 46명의 고혈압이 없는 당뇨병환자에서 magnetic resonance spectroscopy로 측정했을 때, left ventricular (LV) mass to LV end diastolic volume ratio가 정상인에 비해 증가되어 있고, 심근의 지방축적량이 많으며 심근의 에너지 효율이 떨어져 있었다[21]. 또한 46명의 정상 대동맥 협착증 환자와 16명의 당뇨병이 있는 대동맥 협착증 환자에서 수술 시 시행한 생검 결과를 분석했을 때, 당뇨병환자에서 심근의 섬유화 비율이 높았으며, 혈관 주위 AGE 축적도 많았다[22].
결론적으로 정리해보면, 당뇨병은 고혈당과 인슐린저항성에 의한 혈관병증에 의한 허혈성 심근병증이 올 수 있지만, 혈관병이 없어도 지질독성과 AGE의 축적, 칼슘 handling 이상에 의한 심근의 섬유화, 그리고 포도당에서 지방산으로 fuel 의 변화로 인한 에너지 효율의 감소로 인한 심근변화로 인해서 당뇨병성 심근병증이 오게 된다[23,24] (Fig. 1).

당뇨병환자에서 심부전 치료

당뇨병 약제 임상연구들에서 심부전 유병률은 다양하게 보고된다. 최근 신약 당뇨병 약제별 심부전 유병률을 보면 DPP-4 inhibitor는 전반적으로는 증가시키지 않는 것으로 보고되지만, Saxagliptin Assessment of Vascular Outcomes Recorded in Patients with Diabetes Mellitus - Thrombolysis in Myocardial Infarction 53 (SAVOR-TIMI 53)에서 saxagliptin을 투여한 군에서 심부전 위험도가 증가되어 권장되지 않고 있다[25]. Glucagon-like peptide-1 receptor agonist (GLP-1RA)는 약간 감소시키는 연구 결과들이 있으며, SGLT2 inhibitor는 EMPA-REG OUTCOME (Empagliflozin Cardiovascular Outcome Event Trial in Type 2 Diabetes Mellitus Patients-Removing Excess Glucose)과 CANVAS (CANagliflozin cardioVascular Assessment Study), DECLARE-TIM 58 (Dapagliflozin Effect on Cardiovascular Events-Thrombolysis in Myocardial Infarction 58) 연구 등에서 공통적으로 심부전 위험률을 감소시키는 것으로 보고된다[6].
그렇다면 심부전이 있는 당뇨병환자에서의 혈당은 얼마나 낮추어야 할까? 1,447명의 심부전이 있는 당뇨병환자에서 시행한 후향적 연구 결과에서 사망률은 HbA1c가 7∼8% 사이일 때 가장 낮았으며, 미국 재향군인병원 데이터에서 5,815명의 환자를 2년간 추적관찰한 연구에서도 HbA1c가 7.1∼7.8% 사이에서 사망률이 가장 낮은 것으로 보고되었다[26,27]. 2019년 미국심장학회 권고안에 의하면 당뇨병과 심부전이 있는 환자의 혈당 조절은 환자의 임상적, 기능적 상태, 자가관리 능력, 사회적 지원정도, 약제의 부작용과 비용, 저혈당 위험도, 그리고 치료제 burden을 고려해서 개별화해야 함을 권고하였다[16].

당뇨병 약제의 심부전에 대한 영향

다양한 당뇨병 약제의 심부전에 대한 영향은 다른데 계열별로 class effect로 보는 약제도 있고 같은 계열이라도 약제마다 다른 경우도 있다. 이전에 나온 연구들에서 이미 알려진 바대로 TZD는 심부전 환자에서 쓰지 않도록 되어 있으며, 이는 다양한 메타분석 연구에서 공통적으로 보고된다[28]. 메트포르민은 예전에는 심부전 환자에서 쓰이지 못했으나 2006년에 U.S. Food and Drug Administration에서 그 조항은 삭제되었다[29]. 9개의 코호트 연구와 34,000명의 환자에서 분석한 메타분석 연구를 보면 메트포르민 투여는 risk ratio 0.80 (95% confidence interval [CI] 0.74∼0.87)으로 심부전 위험도를 20% 감소시키는 것으로 분석되었다[30].
설폰요소제의 심부전에 대한 영향은 아직 논란이 있는데, 제2형 당뇨병환자에서 집중적 약물요법과 PCI (percutaneous coronary intervention), 혹은 약물요법만 시행해서 심혈관질환 outcome을 본 BARI-2D 연구에서 보면 설폰요소제가 속한 인슐린 보충요법군에서 심부전 위험도가 유의하게 증가하지는 않았다[31]. 또한 최근 발표된 CAROLINA (Cardiovascular Outcome Study of Linagliptin versus Glimepiride in Patients with Type 2 Diabetes) 연구에서 설폰요소제인 glimepiride와 DPP-4 inhibitor인 linagliptin의 심부전에 대한 효과를 비교하였는데, glimepiride군에서 심부전에 의한 입원이 특별히 증가하지는 않았다[32]. 그러나 미국 재향군인 데이터에서 분석한 자료에서는 설폰요소제와 메트포르민을 투여 받은 당뇨병환자에서 설폰요소제를 투여 받은 환자의 심부전에 의한 입원 위험도가 메트포르민군보다 1.3배 증가한 것으로 보고되었다[33]. 그러나 이 연구는 메트포르민과 비교한 것으로, 설폰요소제 자체의 위험요인인지는 확인이 어렵다.
DPP4 inhibitor의 심부전에 대한 효과는 class effect로 결론을 내리기 어렵다. Saxagliptin의 CVOT (cardiovascular outcome trial)인 SAVOR-TIMI 53 연구에서는 saxagliptin 투여군에서 심부전 위험도가 증가하였다[34]. 그러나 DPP4 inhibitor의 CVOT 연구들을 모아서 분석한 메타분석 결과에서는 심부전의 위험도가 증가하지는 않는 것으로 분석되었다[35].
SGLT2 inhibitor는 EMPA-REG OUTCOME, DECLARE-TIMI 58, 그리고 CANVAS 연구 등 이 약제를 투여한 환자들의 CVOT에서 공통적으로 심부전 위험도를 낮추는 것으로 보고되었다[36]. 세 연구를 통합해서 분석한 메타분석 연구에서 SGLT2 inhibitor는 심부전 위험도를 31% 줄이는 것으로 보고된다. 이러한 효과는 이전에 심부전이 있었던 환자나 없었던 환자나 공통적으로 보여서, SGLT2 inhibitor의 심부전에 대한 효과는 매우 강력한 것으로 생각된다. 또한 SGLT2 inhibitor의 심부전에 대한 효과는 신장기능이 낮을수록 더 효과가 좋은 것으로 나와서, 이 약제가 당뇨병환자에서 심장과 신장이 서로 영향을 주는 소위 ‘cardio-renal spectrum’의 악순환을 끊는 역할을 하는 것으로 생각된다[37].
당뇨병환자에서의 SGLT2 inhibitor의 심부전에 대한 유의한 효과는 당뇨병이 없는 심부전 환자에서도 효과가 있을지 의문을 갖게 했다. 최근 발표된 Empagliflozin Outcome Trial in Patients with Chronic Heart Failure with Reduced Ejection Fraction (EMPEROR-Reduced), Study to Evaluate the Effect of Dapagliflozin on the Incidence of Worsening Heart Failure or Cardiovascular Death in Patients with Chronic Heart Failure (DAPA-HF) 두 연구는 심부전을 가진 환자들을 대상으로 SGLT2 inhibitor 를 투여하여 당뇨병이 없는 환자도 포함하였는데, 두 연구 모두 유의하게 심부전의 위험도를 줄이는 것으로 보고하였다[38,39]. 또한 이 두 연구를 합쳐서 분석한 메타분석에서는 당뇨병이 있는 환자나 없는 환자에서 유사하게 SGLT2 inhibitor가 심부전 위험도를 25% 감소시키는 것으로 분석되어, 앞으로 이 약제가 심부전 환자에서 치료판도를 바꿀 수 있는 가능성을 보여주었다[40].
GLP-1RA가 심부전에 미치는 영향은 심근에의 항염증작용과 포도당 이용률 증가, 좌심실 기능 호전, 허혈성 손상 감소 등의 효과로 심근에 긍정적 영향을 미칠 것으로 생각된다[41]. 기존의 GLP-1RA CVOT에서 심부전에 대한 영향은 다양하게 보고되는데, GLP-1RA의 7개의 CVOT를 분석한 메타분석에서는 hazard ratio 0.91 (95% CI 0.83∼0.99)로 약 9% 정도 심부전의 위험도를 낮추는 것으로 분석되었다[42].
이상을 정리해보면, 당뇨병 약제 중에는 SGLT2 inhibitor 가 가장 일관되게 심부전에 긍정적 효과를 갖는 것으로 보고되며, TZD는 쓰면 안될 약물이고 다른 약제들은 다양한 효과를 보인다.

당뇨병 가이드라인에서 심부전에 대한 권고

미국당뇨병학회 가이드라인에서는 몇 년 전부터 심부전이 있는 당뇨병환자에서는 메트포르민 다음 단계로 SGLT2 inhibitor를 처방하도록 권고하였다. 2020년 가이드라인부터는 EF가 45% 이하인 EF가 감소된 당뇨병환자에서 특히 SGLT2 inhibitor를 사용하도록 권고하여 보다 구체적인 권고를 하고 있다[43]. 반면 TZD는 피하도록 권고한다.
2019년 유럽당뇨병학회 가이드라인에서도 역시 당뇨병이 있는 심부전 환자에서는 SGLT2 inhibitor를 쓰도록 class I으로 권고하고 있다[44]. 메트포르민도 class IIa로 심부전 환자에서 권고하고 있으며, GLP-1RA와 DPP-4i는 class IIb로 neutral로 정의하고 있는데, saxagliptin은 class III로 권고하지 않는다. TZD 역시 class III로 권고하지 않는다.
대한당뇨병학회 치료지침에서도 심부전이 있는 당뇨병환자에서는 SGLT2 inhibitor를 쓰도록 권고하며 TZD는 피하도록 권고한다[45]. 미국심장학회에서는 심부전과 당뇨병의 치료에 다학제적인 팀으로써의 접근이 필요함을 강조하고 있으며, 1차 진료의뿐 아니라 사회복지사, 약사, 영양사, 운동치료사, 그리고 신장내과 전문의와 입원전담의의 역할도 필요하다고 강조한다[24]. 이는 당뇨병환자에서 심부전 치료에는 약제 등의 선택뿐 아니라, 운동, 식사요법, 그리고 사회적인 지원 등도 중요하다는 것을 시사하고 있다.

결론

심부전은 당뇨병환자에서 간과되었던 합병증으로 최근 연구들에서 강조되고 있다. 이미 국내연구에서 당뇨병환자에서 심부전 위험도 증가는 알려져 있다. 심장내과에서 심부전 치료에 쓰이는 약제들 말고도 당뇨병환자에서는 심부전이 있을 때 가급적 심부전에 도움이 되는 약제를 선택하는 것이 중요한데, SGLT2 inhibitor가 가장 효과 있는 치료제로 권고된다. 또한 최근 당뇨병 가이드라인에서도 심부전 환자에서 SGLT2 inhibitor를 권고한다.
심장내과로 의뢰되는 증상이 뚜렷하고 EF가 확실히 떨어진 심부전 외에도, mortality나 morbidity가 생기기 전까지 전혀 예측이 되지 않는 무증상 심근병증이 당뇨병환자에서는 이미 많을 수 있다. 또한 전당뇨병 단계에서도 이미 심부전 위험도가 증가되는 바, 늘 이러한 개념을 갖고 당뇨병환자를 진료해야 할 것으로 생각된다.
