Recurrence Rate of Gestational Diabetes Mellitus in Korean Pregnant Women

Abstract
Background
The aim of this study is to determine the recurrence rate of GDM and to find out risk factors for recurrence in pregnant women. Method: This study was conducted on 65 patients whose index pregnancy was complicated by GDM and subsequent consecutive pregnancy was managed at CHA General Hospital between 1996 and 2000. Diagnosis of GDM was based on the criteria recommended by National Diabetes Data Group. During consecutive pregnancy all subjects peformed 100 g oral glucose tolerance test (OGTT) without regard to the result of GDM screening test. Results: Thirty one (47.7%) of the 65 women had recurrence of GDM. Patients with recurrent GDM had higher prepregnancy BMI(22.4±3.4 vs 20.6±1.9㎏/㎡, p=0.011), higher plasma glucose level on GDM screening test (9.9±1.7 vs 8.4±0.9 mmol/L, P=O.000), higher area under the glucose response curve on 100 g OGTT (1694.1± 186.3 vs 1593.4±85.9 mmol/Lㆍmin, p=0.006), delivered heavier baby (3.5 ±0.6 vs 3.2±0.4㎏, p=0.017), and had higher incidence of giant baby (19.4 vs 2.9%, p=0.033) in their index pregnancy compared to patients who had normal subsequent pregnancy. Maternal age, parity, family history of diabetes, gestational age at diagnosis of GDM, and interval between pregnancies did not show significant difference between those two groups. Relative risks for recurrence associated with plasma glucose on GDM screening test ≥9.45 mmol/L, delivery of giant baby in index pregnancy, and weight gain more than 6㎏ between pregnancy were 17.0 (p=0.000), 7.9 (p=0.047), and 9.6 (p=0.046) respectively. Plasma glucose level on GDM screening test was unique independent risk factor for recurrence by logistic regression analysis (Expβ 3.2, 95% CI 1.6-6.3, p=0.0007). Conclusion: Recurrence rate of GDM in this study was 47.7%. Hyperglycemia and heavier infant birth weight in index pregnancy, excessive weight gain between pregnancies were risk factors associated with recurrence of GDM. Among them the most significant risk factor for recurrence was plasma glucose level on GDM screening test in index pregnancy.
